Learning from disasters: how do we share the knowledge and experience?Introduction:Understanding the difficulties of sharing knowledge generated from disaster situations is essential to allowing for a better process of disseminating the u2018lessons learnedu2019 from the ongoing natural and manmade situations which result in healthcare crises.Aim:To explore nurses ways of gaining knowledge from previous experiences, with a particular focus on earthquakes and natural disasters, in the New Zealand (NZ) setting.Method:Initial analysis of a series of individual, semi-structured interviews with a small group (n=10) of emergency department registered nurses from a range of geographical areas in NZ.Results:While familiar with the major earthquake events that have recently affected different areas of NZ (most notably Canterbury, Kaikoura and Wellington), few could recall detailed information or lessons generated from these events. When asked about the most effective means of learning about and from disasters, direct experience was identified as the most effective followed by narrative retelling and vicarious experience.Discussion: Recognition of the value of u2018story-tellingu2019 in sharing information, and of the importance of offering experiences in a way that allows colleagues to experience or place themselves in the situation in a u2018virtualu2019 sense is necessary for learning to occur. This involves an emotional as well as an intellectual connection to occur. There is a risk for knowledge to be lost, and lessons to be constantly u2018re-learntu2019, as each succeeding generation needs direct involvement to retain the information and insight generated. We need to tailor the medium by which this information is shared, for maximum effect.

This series of essays on safety and loss prevention is aimed at helping the process industries avoid accidents and improve its public image. The central message is the apparent inability of organizations to learn, and retain in the long term, the lessons drawn from accidents. Thus incidents of a similar type recur within the same company at intervals of a decade or so, as personnel involved move on to other jobs.

"Using a wide variety of real-world examples such as the Apollo 13 moon mission, Coca-Cola's Dasani water and the Millennium Dome, the book analyzes what those involved in the projects did when things started to go wrong, and how effective - or otherwise - their actions were in improving the final outcome. The tone of the book is positive throughout - the focus is on the identification of root causes and the use of appropriate diagnostic tools and remedial skills and practices to produce the best possible outcome. Due consideration is also given to the use of preventative measures such as risk management to nip problems in the bud before they get out of control."--BOOK JACKET.

Learning From Accidents written by Trevor A. Kletz and has been published by Routledge this book supported file pdf, txt, epub, kindle and other format this book has been release on 1994 with Technology & Engineering categories.
Accident investigation is like peeling an onion. Beneath one layer, of causes and recommendations, there are other, less superficial layers. The outer layers deal with the immediate technical causes while the inner layers are concerned with avoiding the hazards and with weaknesses in the management system. Often only the outer layers are considered and thus we fail to use all the information for which we have paid the high price of an accident. This book aims to show, by analysing accidents that have occured, how we can learn more from them and thus be better able to prevent them occuring again. Looking at a wide range of accidents from the trivial to major disasters and covering the process industries, nuclear industry and transportation, the author analyses each accident in a practical and non-theoretical fashion and summarises each with a chain of events showing the prevention and mitigation which could have occured at every stage. At all times the book emphasises cause and prevention rather than human interest or cleaning up the mess. The chapter on Piper Alpha has been especially written for the book by Brian Appleton, one of the technical assessors of the official enquiry. Anyone involved in accident investigation and reporting of whatever sort and all those who work in industry, whether in design, operations or loss prevention will find this book full of invaluable guidance and advice.
