Findings: Although there were no statistically significant patterns, a number of clinical change concepts emerged as a result of applying Rogers's diffusion of innovation model, including that: Secular trends are critical in predicting practice change; Direct trialability is often low and observability is affected by numbers needed to treat; and Adoption is influenced by the notion of absolute risk for the patient and just one adverse event can have a powerfully dissuasive effect. Conclusion: Rogers's model appears to have some value as a tool to analyse the spread of evidence-based health care recommendations but is more appropriately targeted at truly innovative practices or technologies that are distinct and novel. It may be of most value as a prospective framework for planning change initiatives - to help anticipate and address key barriers.
