One of the most valuable root-cause analysis tools in the system safety toolbox is fault tree analysis (FTA). A fault tree (FT) is a graphical diagram that uses logic gates to model the various combinations of failures, faults, errors and normal events involved in causing a specified undesired event to occur. The graphical model can be translated into a mathematical model in order to compute failure probabilities and system importance measures. A FT can model all aspects of a system, including hardware, software, human actions and the environment. FTs are employed to evaluate large complex and dynamic systems, in order to understand and prevent potential safety and reliability problems. Using the rigorous and structured methodology of FT construction allows the systems analyst to model the unique combinations of fault events that can cause an UE to occur. This book provides an overview of the FTA process; it describes the symbols, terms, construction methodology and mathematics of FTA.

Fault tree analysis is key tool in risk analysis. It is a widely used industry standard with applications in amongst others, the aerospace, automotive, chemical and nuclear industries. Society depends on complex engineering systems such as (nuclear) power plants, airplanes, and data centers, and this dependency is only increasing with developments like smart grids and self-driving cars. In this light, risk analysis is essential to ensure that these systems are reliable. Fault tree analysis is a powerful, commonly used tool for such risk analysis. This book provides a comprehensive overview of what fault trees are and how they are applied, looking at their role in risk management, illustrating techniques for their construction, and describing their analysis in both practical and mathematical terms. Topics and features: Delivers a crystal-clear overview of techniques for building and analyzing fault trees Offers numerous illustrative examples of methods and industry applications Provides solid mathematical descriptions for exact and approximate analysis Includes an overview of modern extensions of fault trees Examines and explains the whole fault tree analysis process, including the application of results With its comprehensive overview of the practice of fault tree analysis, this book is eminently suitable for industrial and academic professionals in risk management. Furthermore, the overview of analysis techniques will be of great interest to researchers working to advance the field.

Fault Tree Analysis And Reliability Block Diagrams written by Jaroslav Menčík and has been published by this book supported file pdf, txt, epub, kindle and other format this book has been release on 2016 with Technology categories.

Fault tree analysis (FTA) strives to reveal all possible sources of critical failures. It starts from the most critical event ("top event") and looks at its reasons, and continues in this way backwards to the initial events leading finally to the failure. So-called fault tree, plotted using the symbols of Boolean algebra can then be used for the construction of a reliability block diagram, which serves for finding the critical way and probability of failure. The principle of Markov analysis is explained as well.

Offshore Risk Assessment was the first book to deal with quantified risk assessment (QRA) as applied specifically to offshore installations and operations. This book is a major revision of the first edition. It has been informed by a major R&D programme on offshore risk assessment in Norway (2002-2006). Not only does this book describe the state-of-the-art of QRA, it also identifies weaknesses and areas that need development.
