Patient empowerment is examined as a multi-dimensional factor influencing the use of diabetes self-management apps. The research design includes three studies conducted in Singapore. Study 1 examines how features of diabetes self-management apps correspond with theoretical indicators of empowerment, as well as app quality. Study 2 uses semi-structured face-to-face interviews with diabetes patients to draw first conclusions about the relevance of empowerment for diabetes app use. Study 3 includes an online patient survey, and uses cluster analytical methods to test the preliminary Study 2 results (typology of app use), as well as binary logistic regression to compare the strength of influence of various anteceding factors on the likelihood of diabetes app use. The studies show that especially the support by private social patient networks and the medical specialties of supervising physicians play a crucial role for technology-supported self-management.

This project is aimed to explore the benefit of diabetes self-management education (DSME) on the self-management of type 2 diabetes mellitus (T2DM). The review of literatures shows the increased concern about the development of type 2 diabetes related complications. The use of diabetes self-management education as revealed by studies helps in a tremendous way in the self-management of type 2 diabetes, thereby helping to reduce the incidence of the complications that occur from poor management of T2DM. The complications when they occur, leads to increased frequency in hospital admission, long hospital stay, high cost of treatment, increased morbidity and mortality rates. Diabetes education has been an ongoing process in the management of diabetes, but the cases of increased hospitalization and development of complications still exist. The aim here is to create more awareness through education for self-management of the condition by the patients with T2DM. The education should be of self-management approach, and not the traditional instructional approach that leaves the patient dependent on the clinicians only for the management of the ill-health. The various literatures reviewed support the use of DSME to achieve this purpose of engaging, empowering, and encouraging the T2DM patients to take control of their diabetes care. The nurses as well as other clinicians can make this happen by reinforcing what is taught and encouraging their patients to adopt the self-management approach. The implication to nursing practice is in the need for nurses to help patients with T2DM to embrace this self-management education approach, and physician/other clinicians should endeavor to refer their patients with T2DM to diabetes classes.

The overall goal of my CNL project was to increase participation of diabetic patients in diabetes self-management education (DSME) by at least 20%, by identifying and addressing barriers and obstacles hindering the achievement or maintenance of diabetes self-management behaviors in a medical-surgical unit. A total of 20 diabetic patients completed both the pre - and pot-test questionnaires; 40% of participants were Asian Americans, 30% were African Americans, 20% were Hispanics, 10% were Caucasians. A pre- test questionnaire was administered to identify the barriers susceptible to hinder the achievement of diabetes self-care behaviors. The main barriers identified include the limited accessibility and the high cost of healthy foods, the inability to read food labels, the lack of knowledge about carbohydrate counting, the lack of social support, the lack of time and lack of motivation, the lack of knowledge of self-care procedures including blood glucose monitoring, medication management, poor problem-solving skills, maladaptive coping mechanisms, and the lack of knowledge of acute complications. Following the pre-test, an intervention consisting of education of patients in diabetes self-management was implemented to address the identified barriers. The goal of the education was to improve the overall health status by empowering diabetes patients to acquire knowledge, learn skills, develop the appropriate behaviors, develop the problem-solving and coping skills to overcome the barriers and obstacles identified. Following the intervention, over 50% of participants agreed to be able to make improvements in diabetes self-management behaviors. Most of them did change their self-management behaviors; others started reflecting on it.
