Because of the wealth of available knowledge, all countries can, at some useful level, implement the four basic components of cancer control -- prevention, early detection, diagnosis and treatment, and palliative care -- and thus avoid and cure many cancers, as well as palliating the suffering. These guides provide practical advice for program managers and policy-makers on how to advocate, plan and implement effective cancer control programs, particularly in low- and middle-income countries. Module 1 shows how to create a cancer control plan that responds to the needs of the people, is based on evidence, and ensures that limited resources are used in an efficient and equitable way, and offers step-by-step advice for developing a cancer control plan. Module 2 investigates how to implement effective cancer prevention by controlling major avoidable cancer risk factors. Module 3 examines how to implement effective early detection of major types of cancer that are amenable to early diagnosis and screening. Module 4 covers how to implement effective cancer diagnosis and treatment, particularly linked to early detection programs or curable cancers. Module 5 shows how to implement effective palliative care for cancer, with a particular focus on community-based care. Module 6 investigates how to advocate for policy development and effective program implementation of cancer control.

In 2005, 7.6 million people died of cancer. More than 70% of those deaths occurred in low and middle income countries. Cancer is low or absent on the health agendas of low- and middle-income countries (LMCs) despite the fact that more people die from cancer in these countries than from AIDS and malaria combined. International health organizations, bilateral aid agencies, and major foundations—which are instrumental in setting health priorities—also have largely ignored cancer in these countries. This book identifies feasible, affordable steps for LMCs and their international partners to begin to reduce the cancer burden for current and future generations. Stemming the growth of cigarette smoking tops the list to prevent cancer and all the other major chronic diseases. Other priorities include infant vaccination against the hepatitis B virus to prevent liver cancers and vaccination to prevent cervical cancer. Developing and increasing capacity for cancer screening and treatment of highly curable cancers (including most childhood malignancies) can be accomplished using "resource-level appropriateness" as a guide. And there are ways to make inexpensive oral morphine available to ease the pain of the many who will still die from cancer.
