In brief, in an experimental animal the ureter of one kidney was ligated and the subsequent renal hypertrophy of its contrala teral mate was studied quantitatively from anatomic and functional stand points. There were two central questions in the Hinman study: How does a normal kidney of an experimental animal recognize that its load has been doubled abruptly? What is the signal for renal hypertrophy? Benign hypertrophy of the prostate is quite different from compensatory hypertrophy of the kidney. It is now known that benign prostatic hypertrophy (BPH) is not a hypertrophy but a benign tumor consisting of a collection of spheroids of micro- and macrodimensions. In technical terms BPH is an adenofibromyoma. Perusal of the present volume will reveal many fascinating facets of BPH of particular interest to urologists and others with an investigative bent of BPH does not occur in children. BPH occurs as a medical rarity in mind.

A symposium on benign prostatic hyperplasia is appropriate at a time when new knowledge and new technology are rapidly emerging. As the age of the population has increased and diagnostic methods have improved, the incidence of diseases of the prostate has increased. Benign prostatic hyperplasia (BPH) is the most common benign tumor in men and results in urinary symptoms in the majority of men older than 50 years; furthermore it has in the past necessitated operative intervention in 20%-30% of men who live to the age of 80 years. The relief of obstruction resulting from this benign neoplasm by means of transurethral resection (TURP) or surgical enucleation represented a major advance and helped to establish urology as a major surgical science. Over the years, urologists became more and more proficient with the resectoscope, reducing the need for open surgery, and over time more procedures were done for more modest symptoms. Although unquestionably effective in the relief of bladder outlet obstruction in the majority of men treated, TURP has not been without morbidity. In 1989, the American Urological Association (AUA) reported an 18% im mediate postoperative morbidity in its cooperative study of 13 participating institutions evaluating 3885 patients (MEBUST et al. 1989). Included were patients requiring transfusion, those experiencing excessive absorption of irrigating fluids ("TUR syndrome"), those experiencing myocardial arrhythmias, and a few suffer ing myocardial infarction.

An intense discussion has recently begun regarding current standards in the diagnosis and treatment of benign prosta tic hyperplasia (BPH). A number of factors have led to this discussion. In an increasing proportion of aging men, for example, BPH causes so-called obstructive symptoms that must be relieved by medical or operative means. This entails an immense social and economic impact in terms of health costs. In addition, recent data indicate the most frequently performed operation for BPH - transurethral resection of the prostate - is associated with a higher risk of death due to cardiac disease than open prostatectomy. Furthermore, studies using the recently developed technique of uro dynamics to assess bladder outflow obstruction reveal that about 20% -30% of patients treated with transurethral resection or open prostatectomy are actually not obstructed. This means that these patients do not receive the most effective therapy.
