Advance Care Planning(ACP)has long been a staple of caring for people with serious illness. Over its history, it has been defined in different ways. Clinicians, researchers, patients, and the public have developed a variety of perspectives about the many aspects of ACP, ranging from the definition to the timing, goals, outcomes, and value of ACP. To better understand the challenges and opportunities for ACP, acknowledge and highlight divergent viewpoints, and examine what is empirically known and not known about ACP and its outcomes, the National Academies of Sciences, Engineering, and Medicine's Roundtable on Quality Care for People with Serious Illness hosted a virtual public workshop, Advance Care Planning: Challenges and Opportunities, on October 26 and November 2, 2020. The workshop explored the paradox of ACP, its evidence base, ways to think differently about ACP, and various approaches to making it more effective.This Proceedings of a Workshop summarizes the presentations and discussions from that workshop.

This thesis explores the perceptions of Japanese community registered nurses towards the use of advance care planning (ACP). The Japanese government has introduced new guidelines for end-of-life care which now includes the concept of ACP. However, unlike Australia, Japan does not have a legal framework to support ACP. Advance Care Planning is indeed a new concept in Japan and a controversial topic among medical professionals. The researcher attended the 7th Advance Care Planning International (ACPI) conference in Rotterdam 2019 to gain knowledge of the various global perspectives on ACP. The 250 participants from 25 countries demonstrated a strong interest and enthusiasm for the benefits of ACP (ACPI, 2019). As a result of attending this conference, it became clear that there is a need for research exploring the various cultural issues involved in the understanding of ACP around the world. From both the researcher's attendance at the conference and her own presentations at public lectures in Japan, it became apparent that ACP is urgently needed as a tool for individual decision-making, and that health professionals should take the lead in talking more openly about ACP. Health professional leadership in this area should support a change in both public opinion and culture in Japan. This study was informed by the qualitative methodology of interpretive description, which involved interviewing 11 community registered nurses. The results showed that the majority of community registered nurses interviewed ACP to be of high importance; however, they were not sure how to use ACP in their role as community registered nurses. There were also difficulties in adopting ACP arising from differing cultural contexts and backgrounds between the use of ACP in Australia and Japan. The results from this study emphasised the need for the establishment of governmental policy, clear documentation, further education, and training in assertive communication. Advance Care Planning is the key to improving the quality of the end of life for Japanese people and it will ensure more information about decision-making in the future. This research may encourage the development of a new policy, establishing both a definition of ACP and standardised documentation.

The fundamental aspects of the way we die has changed with our medical and technological advances. Before the advancement of medicine, people died at home with their loved ones as caregivers. Today, while most people wish to die at home, the majority of people now die in hospitals and extended care facilities after prolonged illnesses. Patients and families now often face health crises, and during these there are times when patients are unable to speak for themselves. Research has shown that advance care planning increases compliance with patient wishes, resolves family and patient distress, and provides a guideline for medical staff to determine appropriate treatment modalities and to respect patient's wishes. Social workers are often trying to mediate the differences between what a family wants and what a patient wants. When advance care planning has not been completed the social workers advocate for patients. In order to increase completion rates and decrease difficult situations facing social workers during crises an advance care planning class was created at St. Joseph's Medical Center in San Joaquin County. This class was hosted by a social worker and a palliative care team which included two RNs and one physician. The overall response was overwhelmingly positive and the main recommendations were to advertise the class more, to offer it at extended care facilities in the area and to offer it year round. Videos to assist the material were shown during these classes and were determined to be exceedingly helpful despite the difficult material.
